Customer Survey Requirement

I am in the process of drafting a customer survey to seek feedback as required by the 2009 standards. Has anyone else started or are doing this now and what did you find, beside posting it on the web site, worked well for getting responses from customers?

We use an online email/survey service called constant contact. It has a monthly fee, but for the low price you can construct surveys to email to your clients.

Write a brief survey that asks feedback questions that you think are of importance for you in terms of improving your laboratory operations.

Include that survey when you send analytical reports to your client, with instructions on how to return the survey to you.
