If a performance test fails do we have to request an additional test to replace the failed results even if it' s the first failure of a particular test? Also, if an additional test is requested to replace the failed result and the additional test doesn't pass, will this cause the lab to have failed 2 out of the last 3 PTs, even though the second failure was an additional PT (not one of the semi-annual PTs)?
